The following 'PRiSM' forms are for use by Devon ACL hourly-paid staff in connection with teaching or non-teaching work. For general information about viewing and downloading files, click here.
Forms to print and fill in by hand:
- Form Prism 7A (pdf, May-2010) for work as a Tutor or Senior Practitioner
- Form Prism 7AN (pdf, May-2010) for work as Tutor Assistant, Kiln Firing Assistant, Art Model, Invigilator, Assessor, Internal Verifier
Excel spreadsheet versions - save FIRST, then fill out using Excel:
- Form Prism 7A (Excel file, May-2010) for work as a Tutor or Senior Practitioner
